Medical Forms

Students participating in Minnesota State High School League (MSHSL) sponsored sports or activities are required to have a physical every three years. If you need a physical, or have had one recently but did not have the form with you, you can download and print it for your physician to complete and return to the school office.

Additionally, all students at St. Croix Lutheran High School should have a medical consent form on file with the school office in the event that medical care is necessary during the school day or at a school sponsored event. Typically, this form is completed and returned with your registration materials. Please update this form if the information changes during the year (i.e., employer or medical provider changes).
